MATERNITY CARE COORDINATION provides case
management to Medicaid eligible pregnant women to help them
define and address priority needs. The Maternity Care
Coordinator may be a Registered Nurse or a Social Worker who
develops a care coordination plan to encourage positive
pregnancy, childbirth, and parenting issues. Referrals are made
to appropriate service providers as needed.
CHILD SERVICE COORDINATION provides emotional support and
counseling to families with children who are diagnosed with, or
at increased risk, for developmental delays. The Child Service
Coordinator is a Registered Nurse or Social Worker whose role is
to identify and follow young children who meet eligibility
requirements. These coordinators also develop a plan of care
with the family to help them identify priority needs and makes
appropriate referrals as needed to help them meet those needs.
MATERNAL OUTREACH WORKERS provide intensive social
support for the expectant mother during her pregnancy, as well
as helping with learning basic child care skills. The Maternal
Outreach Worker continues to support and provide parenting
education during the child's first year of life. The goal of the
MOW program is to help reduce infant mortality.
POSTPARTUM NEWBORN ASSESSMENTS are conducted by a
Registered Nurse who will visit Medicaid-eligible mothers and
their newborn infants within 3 - 7 days of discharge from the
hospital. The RN completes a medical assessment on mother and
baby, as well as providing the families with referrals to
community resources. The nurse answers questions, discusses
self-care for the mother and infant care for the baby. The RN is
trained as a breastfeeding educator and can address problems
that may arise in the first week at home.
SIDS/GRIEF COUNSELING provides a counseling and support
network to Surry County families who suffer the loss of a child
due to Sudden Infant Death Syndrome (SIDS). SIDS counseling in
offered to help families work through their loss and grief, and
to provide information on SIDS, the grief process, and community
resources. SIDS counselors are available for autopsy review and
responsible for sending data to Raleigh for statistical review.
They also provide community education through presentations to
groups, health fairs, etc. They are also available for provide
grief counseling to any family who has lost a child whether
SIDS, miscarriage, or stillborn. CHILD CARE
HEALTH CONSULTANT/SMART START PROGRAM consists of a Health
Education Specialist who provides assistance to child care
providers in obtaining needed in-service education, assists in
developing written policies and procedures concerning health and
safety issues, reviews health and immunization records, makes
appropriate referrals to community resources, and provides
assistance with special needs children. The Health Educator will
conduct assessments of the environment and operations of child
care facilities to identify specific areas of concern. The
Health Educator is trained as an ITS-SIDS trainer and provides
mandated training to all child care providers. |
