Medication Assistance Program 

The purpose of the Medication Assistance Program is to assist seniors and low-income individuals in evaluating, applying for, and completing prescription assistance applications.

Community Alternatives Program 

As the lead agency for the Community Alternatives Program for disabled adults and children, we offer home care as an alternative to nursing home placement.

Chore/Respite Program 

 Services are available to senior citizens age 60 and older who may need assistance with personal care or home management tasks. Respite care is available in order to provide a caregiver with much needed personal time.

Family Caregiver Support Program

The Family Caregiver Support Program provides respite care for caregivers through a consumer driven voucher program.

 

Community Resource Connection

 Information and referral services are available to all who may need assistance concerning resources available to senior citizens or disabled adults.
